I still hope we can figure out some kind of robust gadget (deep funding, prediction markets...) where we can even roughly determine how much some product contributed to agreed-upon public welfare metrics (QALY?), and just replace the patent system with prizes automatically calculated from that. There's a lot of perverse incentives, harm from prevented knowledge sharing, inequality of access, etc that comes from the proprietary nature of the medical industry today.
